Background: Enrolling participants from diverse populations in clinical trials is critical to ensuring that results are broadly generalizable to individuals from communities that face health disparities. Most clinical trials enroll participants who are predominantly white, non-Hispanic, English-Speaking, and from urban communities. The ECHO IDeA States Pediatric Clinical Trials Network (ISPCTN) conducts pediatric clinical trials with a focus on engaging children and families from rural and other medically underserved communities.

Objective: To describe efforts to ensure diverse participant recruitment in a multi-site randomized-controlled vaccine communication trial and the resulting demographic characteristics of enrolled participants.

Design/Methods: MoVeUP is a randomized-controlled trial conducted across 29 clinics within the ECHO ISPCTN. Eligible clinics had to serve patient populations that were either 40% from a rural zip code or < 60% non-Hispanic White. Local site coordinators were trained to request potential recruitment lists from their respective clinics that included the race, ethnicity, language spoken, and zip code of qualifying participants. Coordinators then used these lists to prioritize initial recruitment efforts, focusing first on recruiting individuals from rural zip codes, those listed as race/ethnicity other than non-Hispanic White, and those who spoke Spanish as their preferred language.

Results:

A total of 566 participants have been enrolled in MoVeUP to date. A total of 236 (42%) enrolled participants were non-Hispanic White, 202 (36%) non-Hispanic Black, and 96 (17%) Hispanic (Figure 1). A total of 204 (36%) identified as living in a rural community (Figure 2) and 24 (4%) reported speaking Spanish as their preferred language (Figure 3).

Conclusion(s): MoVeUP has successfully enrolled a diverse cohort of participants, highlighting the effectiveness of targeted clinic and participant recruitment strategies.
