Description: Extremely premature newborns are increasingly surviving with a higher burden of lung-airway-pulmonary vasculature injury via various pre- and post-natal processes. These infants have a spectrum of bronchopulmonary and pulmonary vascular disease that may have a lifelong impact. Recently, there has been an increased awareness for post-natal pulmonary hypertension and the significant impacts of altered pulmonary function and airway anomalies in infants. Neonatologists, with support from multidisciplinary teams, are frequently managing premature infants with pulmonary vascular disease secondary to prematurity in the neonatal care unit (NICU). Once discharged from the NICU, these patients receive regular neuro-developmental follow-up, however cardiovascular follow-up has yet to become standard of care. Prematurity is a major risk factor for cardiovascular disease during childhood and adolescence, and should be acknowledged by teams involved in their care. This session will explore the short- and long-term impact of prematurity on pulmonary and cardiovascular health with experts in the field. The audience will be taking on a hemodynamic and cardiovascular journey from the NICU to young adulthood, with emphasis on risk factors, physiopathology, clinical presentation, screening guidelines and management of pulmonary vascular disease
