Description: Longstanding societal ills - including racism in all its forms - perpetuate child health inequities. To eliminate these gaps, health systems must pursue equity in partnership with communities and identify ways in which to act and learn quickly. In this session, we will discuss an institutional effort to achieve excellent and equitable health outcomes across pediatric conditions. This effort, called the Health Equity Network (HEN), seeks to identify and act on upstream key drivers of equity with strategies that generalize across clinical areas and conditions.
Clinical teams (e.g., those addressing asthma, depression, diabetes, neonatal health) commit to measurably close equity gaps on key outcomes that align with our institution’s strategic plan. Teams participate in longitudinal cross-condition learning activities with one another and with community partners. SMART aims focus on eliminating disparities in outcomes by race, ethnicity, and/or socioeconomic status.
Together, clinical teams focus on 5 key drivers: 1) accountability for equity; 2) capability to provide the right care at the right time and place and in the right way; 3) enhanced positive and addressed negative social determinants of health; 4) effective connections between medical and social care providers; and 5) trusted relationships between providers, patients, and their families.
We will describe our use of quality improvement (QI), advanced analytics, and co-production to create, and grow, a cross-condition learning network within an academic children’s hospital. Each core team employs QI support, actionable data, and community engagement specialists to support testing of targeted interventions based on shared theory and drivers.
