Workshop
Description: Healthcare models that incorporate community health workers (CHWs) into care delivery enable stronger linkages between patients, communities, and health systems. This is due to the unique identities CHWs possess as trusted members of the communities they serve. Their empathy and nuanced understanding of the needs of socially at-risk patients and families confers expertise in delivering patient- and family-centered support. CHWs play a critical role in collaborative multidisciplinary care teams by identifying and addressing health-related social needs, providing linkages to community resources, and strengthening the therapeutic relationships between care team members, and patients and families.
Drawing from a strong body of evidence affirming the need for, and impact of, CHW-integrated healthcare models among adult populations, this workshop aims to highlight the steps needed for successful integration of CHWs within multidisciplinary pediatric healthcare teams. This workshop will be led by clinical, research, and public health experts in implementation science, CHW training and interventions, and development of community-centered family-based pediatric care models. It will provide an interactive experience for participants that fosters peer learning and skill-building.
Those who attend this workshop will enhance their knowledge on the essential roles CHWs play in addressing social risk factors among patients, families and communities; learn specific strategies on how to foster CHW-healthcare provider collaborations; and formulate strategies to overcome potential barriers to the successful integration of CHWs in pediatric care teams. Resultantly, participants will leave with an enriched understanding of CHW-integrated pediatric care models and tangible takeaways to implement these models in various practice settings.
